Mend no longer accepts insurance plans due to their repeated lowering of reimbursement to providers. We are, however, happy to offer a comprehensive Superbill for you to submit for reimbursement. We strive to offer excellent, yet affordable, healthcare to all.
All payments are due at time of services. We have a 24 hour cancellation policy. If your appointment is not cancelled within the 24 hour window there is a $50 Cancellation Fee. If you fail to cancel within 2 hours of appointment time you will be charged the full scheduled treatment fee.
