Forms
All Intake Forms will be sent to you via email upon scheduling an appointment. All forms must be filled out online before your appointment so our practitioner can read all about you and offer you the best care possible. All forms will automatically be uploaded to your electronic chart. We follow all HIPPA rules and regulations and guarantee your complete privacy.
We do require a credit card on file in order to book an appointment. You will not be charged for anything without prior consent or approval. If this is a problem, please advise our office before your appointment.
